Getting Started
Please select the appropriate coverage type below to view the required steps and documents.
- Fill out the Standard Written Order (SWO).
- To meet coverage requirements, please complete and gather the following:
- Local Coverage Determination (LCD) Checklist
- Bain & Findley Activities of Daily Living (BF-ADL) assessment
- A copy of the patient’s medical record
- Fax the completed SWO and all supporting documents to Fasikl Medical Solutions.
- Fill out the Prescription Form.
- Fax the completed Prescription Form to Fasikl Medical Solutions.
